When a business becomes subject to employment insurance, the following documents must be submitted to Hello Work: • Employment Insurance Office Establishment Notification (雇用保険適用事業所設置届) • Insured Person Qualification Acquisition Notification (雇用保険被保険者資格取得届) for each employee • Copy of Labor Insurance Establishment Notification (労働保険 保険関係成立届の控え) • Certificate of Registered Matters (登記事項証明書) • Worker roster (労働者名簿) • Copy of employment contract (雇用契約書) The Labor Insurance Establishment Notification must be filed with the Labor Standards Inspection Office first. Not required for officer-only companies with no employees.
