Overview

Employers must provide regular health examinations (健康診断) for all regularly employed workers at least once per year. The employer bears all costs. Workplaces with 50 or more regular employees must file a results report (定期健康診断結果報告書) with the Labor Standards Inspection Office (労働基準監督署).

The required examination items for the regular health examination are: • Medical and occupational history • Subjective and objective symptom examination • Height, weight, abdominal circumference, vision, and hearing • Chest X-ray (and sputum test) • Blood pressure measurement • Anemia test (red blood cell count, hemoglobin) • Liver function test (GOT, GPT, γ-GTP) • Blood lipid test (LDL cholesterol, HDL cholesterol, triglycerides) • Blood sugar test • Urinalysis (sugar, protein) • Electrocardiogram (ECG) Some items may be omitted at the physician's discretion based on age or previous results. If a pre-employment health examination (雇入れ時の健康診断) was conducted within the past year, the same items may be omitted from the regular examination. The examination is the employer's obligation; all costs are borne by the employer. Even if an employee refuses the examination, the employer's duty is not waived. Workplaces with 50 or more regular employees must submit the Regular Health Examination Results Report (様式第6号) to the jurisdictional Labor Standards Inspection Office.

Penalties for Non-Compliance

Failure to conduct the regular health examination is punishable by a fine of up to ¥500,000 under Industrial Safety and Health Act Article 120. Workplaces with 50+ regular employees that fail to submit the results report (Form No. 6) are also subject to the same penalty. Failure to record and retain examination results (retention period: 5 years) or to consult with a physician regarding results is also subject to penalties.
